Our Transformation Projects
In Shropshire, Telford and Wrekin, we are investing in vital improvements to our services and facilities to ensure all of our communities can access the right treatment, in the right place, at the right time, to help them live healthier, happier, and more rewarding lives.
Our community and hospital services cannot deliver the quality of care our residents need, nor are they financially sustainable. We need to respond to the increasing pressure on services by exploring new models of care and delivering more joined-up health and care services.
We have two major transformation programmes: the Local Care Transformation Programme (LCTP) and the Hospitals Transformation Programme (HTP). Collectively, these two programmes aim to reshape the provision of health and care services across the county.
By integrating NHS and local authority services and creating more joined-up, high-quality care across all settings, we will improve access to care, improve health and wellbeing, enhance patient experiences and reduce inequality.
